磷脂抗体阳性红斑狼疮患者周围血T细胞受体基因表达初探 |

摘 要 目的：初步探讨SLE患者周围血T细胞受体Vβ基因是否存在偏移。方法：应用荧光PCR定量方法对HLA-DRB1*0803／DQA1*0103的抗磷脂抗体阳性SLE患者4例，病人对照干燥综合征(SS)1例，正常对照3例进行周围血TCR-Vβ的分析。结果：健康者和病人对照的TCR仅少数Vβ(＜5个Vβ基因)有轻微增高表达。1例非活动性SLE患者与对照组相仿，但3例活动性SLE的TCR-Vβ基因谱呈多克隆激活，其中2例有15～17个Vβ基因呈中度以至高度表达，甚至Vβ／Cα高达100.5%(Vβ4／Cα)和208.8%(Vβ2／Cα)。3例活动性SLE的Vβ7、Vβ12、Vβ22都呈现异常增高表达，与正常对照有显著差异(P＜0.05～0.01)。结论：选择性的TCR-Vβ基因的优势表达可能对SLE的发病和活动起极大的作用。

A primary study on TCR-Vβ usage of peripheral blood lymphocyte patients with positive antiphospholipid antibody of systemic lupus erythematosus

Abstract Objective: The preliminary inguirng into probable divergence TCR-Vβ usage of peripheral blood lymphocytes in patients with SLE. Methods: 4 patients with SLE, 1 patient control with SS, according to the Amenican college of rheumatology criteria, 3 normal controls were analyzed for T cell Vβ repertoire using fluorecent quantity PCR technique, all cases with positive antiphospholipid antibody and HLA-DRB1*0803/DQA1*0103. Results: The three healthy and one patient (SS) controls showed faint expression in only one to five Vβ families respectively. The patient with non active SLE showed a similar result to the controls, while the other three, who had active disease, showed marked polyclonal activity in their T cell Vβ repertoire, 2 of 3 affecting more than 15 of the 23 Vβ families analyzed. Some Vβ expression was very high even Vβ4/Cα and Vβ 2/Cα with 100.5% and 208.8% respectively. 3 cases of active SLE showed marked abnormal expression of TCR with Vβ7, Vβ12, Vβ22.
