【摘要】 目的：探讨切开复位内固定治疗Pilon骨折的手术疗效。方法：对26例Pilon骨折患者的临床资料进行回顾性分析，采用切开复位内固定的方法对26例患者进行治疗，评价治疗效果。结果：26例患者中优6例，良13例，差7例，总优良率达73.1%。4例出现伤口并发症，7例术后出现创伤性关节炎。结论：切开复位内固定治疗Pilon骨折效果良好，病残率较低。充分的术前准备和计划，熟练、精细的操作技术，固定可靠的材料，早期功能练习是获得满意疗效的基础和保证。

Analysis of the effect of the openreduction and internal fixation of 26 cases of Pilon fractures

Abstract： Objective: To Study the curative operating effects of the pilon fractures treated with the method of openreduction and internal fixation. Methods: By retrospectively analyzing the clinical documents of 26 pilon fracture patients treated with the openreduction and internal fixation and by evaluating the top quality rate and the complications of the operation. Results: Among 26 patients, 6 were excellent, 13 good, 7 poor and 4 patients with postoperation complications. The total top quality rate was 73.1%. Conclusions: There are good effects and the disabling rate is quite low to cure pilon fractures by adopting the method of the openreduction and internal fixation. The adequate preoperative preparation and plan, the skillful and delicate operating techniques, the fixed and reliable materials and the early functional exercise are bases and conditions to obtain the satisfactory effects.

Pilon骨折指波及负重关节面和干骺端的胫骨远端骨折，常合并腓骨骨折，约占下肢骨折的1%，胫骨骨折的3%～10%，临床治疗困难，并发症及病残率高[1]。本文对我院1995～2005年收治的26例Pilon骨折的临床资料进行回顾性分析，以探讨切开复位内固定治疗Pilon骨折的疗效，现报道如下。
1资料与方法
1.1临床资料
本组26例Pilon骨折患者，男性19例，女性7例，年龄16～57岁，平均32岁。骨折原因：坠落伤18例，交通事故伤5例，滑雪摔伤2例，重物砸伤1例;其中闭合性骨折21例，开放性骨折5例;合并腓骨骨折12例;受伤至接受手术的时间为2 h～7 d;按RuediAllgower分型[2]：Ⅱ型损伤17例，Ⅲ型损伤9例。
1.2方法
26例Pilon骨折均采用切开复位内固定的方法进行治疗，
