醋酸亮丙瑞林微囊型缓释剂治疗中枢性性早熟临床观察 |

【摘要】 目的 观察醋酸亮丙瑞林微囊型缓释剂治疗儿童中枢性性早熟的疗效。方法 以醋酸亮丙瑞林缓释剂60～80μg/（kg.次），对15例女性中枢性性早熟患儿进行皮下注射治疗。 结果 治疗2个月后所有患儿第二性征均开始明显改善，2例月经来潮的患儿停止了行经，6个月后，5例患儿TannerⅢ期乳房恢复了正常，黄体生成素（LH）峰值（中位数）由32.9 IU/L降至8.1 IU/L，卵泡刺激素峰值由（21±6）IU/L降至（7±4）IU/L，雌二醇峰值由89.2 pmol/L降至21.7 pmol/L，骨龄无明显增加，骨龄/年龄比下降;预测成年期身高(13例)由治疗前（151.2±3.2）cm升至（153.4±2.8）cm (P<0.01)，皮质醇、生长激素、甲状腺素等治疗前后无明显变化(P>0.05)，治疗中无明显不良反应。结论 醋酸亮丙瑞林微囊型缓释剂能有效地抑制垂体促性腺激素和性腺激素分泌，控制性征发育和延缓骨龄，临床使用安全、方便。

【Abstract】 Objective To evaluate the effect of sustained release leuprorelin acetate in depot (LAD) in 15 female idiopathic central precocious puberty (ICPP) patients. Methods Fifteen patients with ICPP were treated with LAD subcutaneously at monthly intervals. Results After 2 months of treatment, the amount of breast tissue decreased in all patients, menstruation ceased in 2 patients who had menstruation before treatment. After 6 months of treatment, breasts became almost normal in 5 patients whose breasts were in stage Ⅲ before treatment, breasts fell to stage Ⅲ in 3 patients from stage Ⅳ . Mean peak level of serum LH, FSH, E2 decreased significantly. Bone age did not increase. The ratio of bone age/chronological age declined. Predicted adult height increased from pretreatment (151.5±3.2) cm to (153.4±2.8) cm (P＜0.01). No significant difference was found in serum cortisol, GH, T, T3, T4, TSH and PRL between pre- and post treatment levels (P＞0.05). No evidence of side effect was seen except for one patient who had a small amount of bleeding 1 week after the first injection.
