达那唑和促性腺素释放激素类似物对ICPP女性患儿生长和促生长素轴的影响 |

【摘要】 目的 了解不同的性发育抑制性药物对特发性中枢性性早熟(ICPP)女性患儿生长速度、生长激素结合蛋白(GHBP)及胰岛素样生长因子1(IGF-1)的影响。方法 采用对青春期生长呈不同影响的达那唑和促性腺素释放激素(GnRH)类似物，共治疗ICPP女性患儿23例，对比治疗前后患儿的生长速度、血清GHBP、IGF-1和性激素的改变。结果 两药均能抑制副性征。15例经达那唑治疗后，生长速度从(6.8±1.5)cm/年增至(8.4±1.6)cm/年，不伴骨龄加速；血GHBP从(7.2±3.0)% 降至(4.6±2.7)% ，血IGF-1从(11.1±2.9)mol/L增至(12.5±3.5)mol/L、阴道涂片表层细胞数从(28.2±2.7)% 减少至(12.0±2.0)%，血睾酮从（0.5±0.4）nmol/L升至（1.0±1.0）nmol/L，但仍在女孩青春期正常范围内，尿生长激素无明显改变。8例经GnRH类似物治疗后，生长速度从（7.8±3.3）cm/年降至（5.6±1.8）cm/年，骨龄增长速度减慢一半，血GHBP从(6.0±2.7)% 增为(8.0±2.3)% ，血IGF-1从(15.8±2.7)mol/L显降至(10.6±3.0)mol/L、血雌二醇从(64±10)mol/L降至(21±5)pmol/L、血睾酮从(1.42±1.14)nmol/L显降至（0.48±0.20）nmol/L。结论 达那唑和GnRH类似物都能使ICPP女性患儿的血雌二醇下降，并抑制性征，但两者对生长速度，血GHBP、IGF-1、睾酮水平的影响是相反的。

【Abstract】 Objective To recognize the changes in growth velocity, serum levels of growth hormone binding protein (GHBP) and insulin-like growth factor-1(IGF-1) in patients treated with different medicines to inhibit the precocious sexual development. Methods The girls with idiopathic central precocious puberty (ICPP) were treated with Danazol or GnRH analogue, which have different effect on puberty growth. The changes of the growth velocity, GHBP, IGF-1 and sex steroids were observed during treatment. Results The results showed that both drugs could inhibit secondary sexual characteristics.
