低位腹主动脉间歇性阻断技术的临床应用 |

【摘要】 目的 回顾低位腹主动脉间歇性阻断技术在骶骨肿瘤切除、骨盆严重损伤合并其它器官裂伤修复术中的临床应用效果。方法 术中应用消毒皮管加粗鞋带套结法间歇性阻断腹主动脉9例。结果 术中失血量得到有效控制，输血量减少，手术野清晰，缩短了术中止血时间，且生命体征较稳定。术后观察及长期随访未发现因血管阻断而出现的各种并发症。结论 低位腹主动脉间歇性阻断技术，操作简便，血流阻断范围广，技术可靠，能有效减少失血量。

Clinical use of intermittent block of lower abdominal aorta

【Abstract】 Objective To review the clinical effect of the intermittent block of lower abdominal aorta in resection for sacral tumor and treatment of serious pelvis injury complicated with ruptured visceras. Methods 9 patients lower abdominal aortas were intermittent strapped and blocked by sterilized rubber tube and shoelace during operations. Results The technique could evidently decrease bleeding and blood transfusion during operations. The field of vision in operations was clear，the stanching bleeding time was shortened and the life sign was stable. The patients were followed up postoperatively for a long time and no complications occured because of the block. Conclusion The technique of the intermittent block of lower abdominal aorta is easily handled.The range of blood blocked is wide . The technique is reliable，and can reduce hemorrhage effectively.

骨盆部位的骨肿瘤常因瘤体大且与周围血管、神经、盆腔脏器粘连，故手术创面大，容易误伤，且分离部位深而引起较多的出血，给手术切除肿瘤增加一定的风险。严重骨盆骨折合并其他脏器的损伤，则因快速大量失血难以控制，有危及生命的危险〔1〕。间歇性阻断腹主动脉，可较大范围地阻断血流，减少失血量〔2〕，使手术野清晰，手术操作时间缩短，增强手术的安全性。作者1995年5月～1999年9月，选择性采用该项技术分别进行7例骨盆肿瘤的切除及2例骨盆严重骨折伴盆腔脏器损伤和失血性休克的救治，取得满意效果。
1 材料与方法
1.1 病例资料 本组9例，男6例，女3例，年龄7～52岁。骨盆严重损伤合并盆腔脏器裂伤、失血性休克2例；骨盆部位肿瘤7例，其中脊索瘤5例（其中1例系肿瘤切除后5年复发者），骨巨细胞瘤1例，髋骨巨大软骨肉瘤1例。
1.2 治疗方法 常规气管插管全身麻醉。除骶骨肿瘤切除需侧卧位，其他患者均仰卧位，垫高腰骶部。选术侧倒八字切口，切开皮肤、皮下组织、腹外斜肌、腹内斜肌、腹横肌及筋膜，自腹膜外脂[1] [2] 下一页
