［摘要］ 目的 探讨腰麻-硬膜外联合麻醉（CSEA）在经皮穿刺125I植入治疗前列腺癌中的临床价值及可行性。方法 手术患者40例，年龄58～81岁，平均73.5岁，ASA Ⅱ～Ⅳ级。随机分为CSEA组（A组） 和EA组（B组），每组20例，A组蛛网膜下隙注入布比卡因7.5 mg，平卧测麻醉平面，在硬膜外逐加2％利多卡因达麻醉平面T8，B组常规用药麻醉平面达T8。结果 A组麻醉起效快，麻醉效果满意，无牵拉痛，BP平稳；B组有8例（40％）患者出现麻醉阻滞不全及牵拉痛，血压下降≥30％。结论 CSEA术中麻醉效果满意，用药量小，对血流动力学影响小，是老年患者首选的麻醉方法之一，尤其在经皮穿刺植入125I治疗前列腺癌方面便于操作。

［Abstract］ Objective Outside the discussion waist hemp-hard membrane unites the anaesthe sia（CSEA） in to pass through skin puncture 125I to implant treatment prostate gland cancer the clinical value and the feasibility.Methods 40 examples patients，age 58～81 year old，average 73.5 years old.The ASAⅡ～Ⅳ level stochastically grades CSEA group （B group），each group of 20 examples.A group of arachnoid under cracks pour into the clothcompared to kain 7.5 mg，the horizontal side plan gradually adds 2% outside the hard membrane lidocaine to reach anaesthesia plane T8，B group of conventions reach T8 with the medicine anaesthesia plane.Results A group anaesthetizes the effect quickly，anaesthesia effectsatisfied，not pulls the pain，BP is steady; B group has 8 examples（40%） the patient to appear the anaesthesia to hinder notentire and pulls the pain，the blood pressure drops 30%.Conclusion In the CSEA technique anaesthetizes the effect satidfiedlly，the amountused is small，affects slightly to theblood stream dynamics，is one of old age patient first choice anaesthesia methods，especially impasses through skin puncture 125I to implant the treatment prostate gland cancer in the surgery to be advantageous fou opretion.
