纵隔畸胎瘤的CT诊断

　　［摘要］目的 探讨纵隔畸胎瘤CT征象的诊断价值与限度。方法 从手术后病理证实为纵隔畸胎瘤的85例病人中选择CT资料完整者63例，均经CT平扫和增强扫描。结果 基本CT均表现为孤立性前纵隔包块。其中囊性包块26例，均为成熟型，包括单房囊型13例，多房囊型7例，子-母囊型6例。囊实性包块29例，实性包块8例，均为不成熟型畸胎瘤。特殊CT征象包括:脂-液平面6例，脂密度子囊5例，水密度子囊3例。结论 前纵隔的囊性或囊实性包块内水样密度、脂质密度、软组织密度及不同形状的钙化并存或相互混杂或（和）包块内含有骨块、牙齿，显示脂-液平面、脂密度子囊、水密度子囊对本病有特异性诊断意义。

　　CT DIAGNOSIS OF MEDIASTINAL TERATOMA

　　［ABSTRACT］ObjectiveTo assess the value and limitations of computed tomography （CT） signs in diagnosing mediastinal teratoma. MethodsThe complete CT data of 63 patients were selected from 85 postoperative patients with mediastinal teratoma confirmed pathologically. Plain and enhanced CT scans were performed for all these patients. ResultsThe basic CT manifesta-tions of the teratoma were as follows: 26 cases appeared as an isolated mature cystic mass of anterior mediastinum， which included 13 with monocystic， seven with multicystic， six with filial-maternal-cystic; 29 showed cystic-solid mass; eight with solid mass. Some special CT signs were fatty-fluid interface in six cases， fatty density filial cyst in five， and water-density filial cyst in three. ConclusionThe CT features of diagnostic significance of mediastinal teratoma are water-density， fat-density， soft tissue-density and different shape calcification coexist or intermix in the isolated mass in anterior mediastinum， and/or bone mass， tooth， fatty-fluid interface， fatty density filial cyst， water-density filial cyst in the isolated mass.

　　纵隔畸胎瘤CT表现常复杂多变，虽然多数病例可在术前根据CT表现作出正确诊断，但误诊率仍较高。本文回顾性分析我院1990～2005年间确诊的63例纵隔畸胎瘤的CT表现，旨在探讨其特殊CT征象的诊断价值与限度。
