肝脏与胃肠道同时存在肿块的处理原则 附12例报告 |

【摘 要】 目的 总结肝脏与胃肠道同时存在肿块的处理原则。方法 分析12例肝脏与胃肠道同时有肿块而术中或术后病理检查证实并非癌肿转移关系者的临床资 料。结果 7例为同时性双重癌，3例为肝脏癌肿并胃肠道良性疾病，1例为结肠癌伴套迭而肝脏病灶来 自胰腺囊实性肿瘤(低度恶性)，另1例肝脏与胃肠道均为良性疾病。 结论 肝脏与胃肠道同时存在的肿块并不一定是癌肿转移，可能为同时性双重癌或伴发良性肿物； 非转移病变的治疗与转移性癌截然不同，强调首先获得病理诊断，以进一步选择相应的手术 方式。

The principle of management for synchronous masses in the liver and gast rointestinal tract(report of 12 cases).

【Abstract】 Objective To summarize the principles of management for synchronous masses in the liver and gastrointestinal tract.Methods The clinical data of 12 cases who had non-metastasis synchronous masses in the l iver and GI tract were analyzed. Results The pathological diagnoses were:7 cases of double synchronous multiple primary m alignant tumor of the liver and GI tract,3 cases of malignant tumors of the live r and benign tumors of GI tract,1 case of colon cancer and cystic-solid neoplasm of pancreas with liver metastasis,the last one was both benign masses of GI t ract and liver.Conclusion Synchronous mass of the liver and GI tract may not be GI cancer metastasis to th e liver,it maybe the synchronous multiple primary malignant tumor or accident be nign tumor.So it is important to select the correct treatment according to the p athological diagnosis.

胃肠道肿瘤可通过门静脉转移至肝脏。反之，肝癌向胃肠道转移极为罕见。当肝脏与胃 肠道同时存在肿块时，通常被认为胃肠道肿瘤伴肝脏转移。然而，临床上有不少例外，作者 近年遇到12例肝脏与胃肠道同时性肿块并非癌肿转移，现报告如下，以进一步探讨胃肠道与 肝脏同时性肿块的适当处理。
1 临床资料
本组男8例，女4例。年龄14～70岁，平均50岁。2例术前发现肝脏与胃肠道均有肿块 而诊断为胃肠道肿瘤伴肝脏转移，术中发现为双重癌。7例术前诊断为肝脏肿瘤而手术中探 查发现伴有胃肠道肿物(其中3例双重癌，1例低度恶性肿瘤，3例良性病变)。3例术前诊断为 肠道肿瘤术中发现伴有肝脏肿块(其中2例双重癌，1例为胰腺低度恶性肿瘤伴肝转移)。详见 表1。
2 讨论
2.1 认识肝脏与胃肠道同时非转移性肿块的临床意义
临床上常用“一元论”把肝脏与胃肠道同时存在的肿块看作胃肠道癌肿伴肝脏转移，归于 晚期肿瘤之列，这在相当程度上影响手术者对其治疗的态度和决策，导致不同的治[1] [2] 下一页
