期乳腺癌的外科治疗 |

［摘要］ 目的 探讨Ⅲ 、Ⅳ期乳腺癌的外科治疗方法。方法 对120例Ⅲ、Ⅳ期乳腺癌的外 科治疗资料进行总结，并随访疗效。结果 行改良根治术的88例Ⅲ 期患者总生存率为84.1%，无复发生存率为79.5%。行根治术的20例Ⅲ期患者总生存率为80%( χ2 =0.196, P>0.05)，无复发生存率为70%(χ2 =0.859, P>0.05)。 对孤立远处转移的Ⅳ期患者采用根治性外科治疗，取得了较好效果。结论 Ⅲ期乳腺癌应以外科治疗为主，多选择改良根治术。对孤立远处转移的Ⅳ期患者， 仍应采用根治性手术治疗。

Surgical therapy of stage Ⅲ and Ⅳ breast carcinoma

［Abstract］ Objective To discuss the surgical therapy a bout stage Ⅲand Ⅳ breast carcinoma. Method Therapeutic da ta of 120 patients with breast carcinoma on stageⅢor Ⅳ was summarized,and the patients were followed up.Results The overall survival rate (OS) of 88 stage Ⅲ patients who received modified radical mastectomy was 84.1%, relapse-free survival rate(RFS) was 79.5%,but in 20 stage Ⅲ patients wh o received radical mastectomy,the OS and RFS were 80% and 70% respectively.The s tage Ⅳpatients who had isolated metastasis were treated by radical surgical the rapy,the effect was better.Conclusions The stageⅢ patients with breast carcinoma should be mostly treated by surgical therapy,and modified radical mastectomy should be the initial choose. The stage Ⅳpatients who had i solated metastasis should be treated by radical surgical therapy.

随着对乳腺癌生物学行为的逐步认识，乳腺癌的外科治疗模式发生了一些 变化，现就我们对Ⅲ、Ⅳ期乳腺癌外科治疗的认识、体会以及手术技巧的改进报告如下。
临床资料
120例患者为 1996年1月～1998年12月收入我院住院经手术治疗的乳癌患者，均为女性。平 均年龄 47.4岁 （ 26～76岁）。TNM分期：Ⅲ期110例，Ⅳ期10例。病理类型：导管内癌4例 ，浸润性小叶癌 3例，浸润性导管癌106例，粘液癌4例，髓样癌3例。手术方式：110例Ⅲ期 患者中行改良根治术88例，根治术20例，减瘤术2例；10例Ⅳ期患者中行改良根治术或根治 术加颈淋巴结清扫4例，乳腺癌伴脑转移先后行脑转移癌切除和左乳癌改良根治术1例，减瘤 术5例。所有患者术后给予放疗和化疗。其中15例(Ⅲ期8例,Ⅳ期7例)术前采用ACF（阿霉素3 0 mg，环磷酰胺400 mg，氟脲嘧啶500 mg）方案进行辅助化疗，[1] [2] 下一页
