乳癌根治术后同期假体置埋隆乳术 |

摘要：目的 报告乳癌根治术后，应用硅凝胶乳房假体同期 置埋隆乳。方法 对手术的方法选择及实施，切除物体积与假体型号 的关系，术后注意事项等问题进行探讨。结果 随访感到手术后乳房 多项外在指标均较满意。结论 该方法比应用各种自体组织隆乳大为 简化，并免除手术分期进行带来的问题。

Augmentation mammoplaty by one stage implant of breast prostheses after radical operation of breast cancer

Abstract: Objective This paper reported the results of au gmentation mammoplasty by using implant of silicon gel breast prostheses simult aneously after radical operation of brest cancer. Methods We discusse the choice of operation mode and the relation between the size of resected breast and breast prostheses model as well as some attentions of operat ion. Results Indices of reconstructed breast were satisfa ctory during follow up. Conclusion This method is more si mple than many others and may avoid many problems due to staged operation.

1 资料与方法
我院自1996年6月至1999年8月，共行乳癌根治术同期硅凝胶乳房假体置入隆乳8例，效果满 意。减轻了患者既往术后一系列精神和肉体上不适。8例乳癌根治术后同期假体置入，临床 资料(表1)。手术方法均行Pater手术，保留原有乳头乳晕，硅凝胶假体置入胸大肌下。

2 手术操作
2.1 设计 首先按手术侧乳房有的外在解剖结构标志及对照 对侧正常乳房的外形结构，在患侧用龙胆紫标出术侧乳房的边界及假体置埋需分离的范围， 同时确定需要切除乳房上皮肤的范围［1］。设计稍保守一点，术中再酌情处理。
2.2 肿瘤切除 以肿瘤为中心，尽可能避开乳头乳晕，在充 分切除肿瘤并顾及腋窝淋巴结清扫的同时做梭形切口。对于保留的皮肤区域应在皮下脂肪层 进行分离，尽量不要破坏胸外侧动脉外侧支和胸廓内动脉的内侧皮支及[1] [2] 下一页
