小剂量多巴胺对SMAO休克兔腹腔脏器氧供氧耗的影响 |

摘要 目的:研究小剂量多巴胺对SMAO休克兔腹腔脏器DO2、VO2的影响。方法:20只家兔随机分成两组(A、B),两组均被诱导成SMAO休克。A组给予多巴胺治疗,B组作为对照组。分别测定血流动力学、动脉及门静脉血乳酸浓度(ALT、PLT)。并分别计算出整体及腹腔脏器DO2、VO2(TDO2/TVO2、SDO2/SVO2),各自的氧摄取率(TO2ER、SO2ER)。结果:模型复制后60分钟开始,两组相比较,A组QpvI、SDO2/SVO2均较B组增加明显(P<0.01),ALT、PLT在90分钟时明显下降(P<0.05),但CI、TDO2/TVO2、MAP等在整个实验期间两组均无显著性差异(P>0.05)。结论:小剂量多巴胺可增加SMAO休克兔腹腔脏器血流灌注量,从而改善了该部位的氧合状态,但对整体的氧合状态则无明显影响。 关键词 氧供 氧耗 多巴胺 SMAO休克
The effect of low-dose dopamine on splanchnic oxygen delivery/oxygen consumption on in rabbits SMAO shock Li Libin,Kuang Xian. Intensive Care Unit,Second Affiliated Hospital of Zhejiang Medical University,Hangzhou 310009 Abstract Objective:To study the effect of low-dose dopamine on splanchnic DO2/VO2,in rabbit SMAO shock Method:Twenty rabbits were randomly devided into two groups:group A with dopamine,group B as control group.In group A dopamine was infused at a rate of 5 μg*kg-1*min-1.SMAO shock model was induced in both groups.Cardiac output index(CI),portal venous blood flow index (QpvI),mean arterial pressure (MAP),arterial,artial and portal venous blood gas analysis,arterial and portal venous serum lactate concentrations(ALT,PLT),total oxygen delivery/oxygen consumption(TDO2/TVO2),splanchnic oxygen delivery/oxygen consumption(SDO2/SVO2),oxygen extraction ratio(O2ER) were measured respectively.Result:QpvI,SDO2,SVO2 were higher in group A than those of group B (P<0.05),ALT and PLT of group A were lower than those of group B (P<0.05) 60,90 and 120 minutes following shock respectively,but before 60 minutes,these values were similiar in two groups (P>0.05).CI,MAP,TDO2/TVO2 decreased,TO2ER and SO2ER increased in both of groups during shock,but without significant differences between both groups.Conclusion:Low-dose dopamine has beneficial effects by increasing portal venous blood perfusion and improving splanchnic oxygenation during SMAO shock in rabbits,but no effects on systemic oxygenation of shock.
