共刺激因子CD80在强直性脊柱炎肠道的表达 |

【摘 要】 目的 研究共刺激因子CD80肠道表达及其与强直性脊柱炎发病的关系。方法 40例强直性脊柱炎病人，其中20例处于病情活动期，20例处于病情静止期，和 40例正常对照肠镜活检取材后用B7/BB1单抗检测CD80在肠道的表达。结果 强直性脊柱炎肠道50%有CD80表达，正常对照仅有2.5%表达，两者比较P＜0.0 01；活动期强直性脊柱炎肠道90%有CD80表达，而非活动期组中仅有10%表达，两者相比， P值＜0.001。结论 共刺激因子CD80肠道表达与强直性脊柱炎发 病有关，与强直性脊柱炎病情活动有关。

【Abstract】 Objective To study the costimulat ory factor CD80 gut expression in ankylosing spondylitis and its association wit h the pathogenesis of ankylosing spondylitis.Methods Forty anky losing spondylitis (20 in active stage and 20 in nonactive stage) and 40 normal controls were included and their CD80 gut expression was probed by monoclonal an tibody B7/BB1.Results CD80 gut expression was found in 50% of a nkylosing spondylitis,while only in 25% of normal controls (P＜0.001).Ninet y percent of the active ankylosing spondylitis had CD80 expression in the gut,wh ile only 10% of the nonactive had (P＜0.001).Conclusion Co stimulatory factor CD80 gut expression is associated with the pathogenesis of an kylosing spondylitis and the disease activity.

分子模拟学说认为强直性脊柱炎发病与肠道有关［1］，但是其具体的分子免疫机制却不甚明了。最近发现一个与机体免疫激活有决定意义的分子B7-1(CD80)，CD80是一个共刺激因子，它只表达于抗原递呈细胞上，而且只限于激活的抗原递呈细胞上［2］。 它的表达意味着免疫激活，它无表达意味着免疫失活［3］。而其与强直性脊柱炎、强直性脊柱炎肠道的关系，尚无文献报道。本文拟通过研究CD80在肠道的表达，研究其与强直性脊柱炎发病的关系。
1 材料和方法
1.1 实验对象：40例强直性脊柱炎病人，男35例，女5例，年龄最小19岁，最大33岁，平均年龄(28.6 ±7.3)岁。强直性脊柱炎诊断标准使用修正的纽约标准［4］，其中活动期和静止期各20例，强直性脊柱炎病情活动根据文献判定［5］。对照组由40例健康体检者组成，男34例，女6例，年龄最小21岁，最大35岁，平均年龄(30.3±6.2)岁。所有病人与对照均在肠镜下活检钳取结肠组织1块。所有实验对象近1个月内未使用激素以及免疫抑制剂。
1.2 实验方法：免疫组化采用LSAB(labelled streptavidin biotin，生物素标记的链霉 卵白蛋白)试剂盒，该试剂盒购自DAKO公司。
