电子束CT测定的冠状动脉钙化与冠心病危险因素的关系 |

【摘要】 目的 探讨电子束CT(EBCT)检测的国人冠状动脉钙化(CAC)与冠心病危险因素之间的关系及其在冠心病预测和预防中的作用。方法 将经Imatron C-150型EBCT机检查CAC的718例患者按年龄、性别和拥有冠心病危险因素多少分组。分析男、女性CAC率与血压、血脂、糖尿病、家族史、吸烟等冠心病危险因素的关系、拥有冠心病危险因素多少对CAC率和积分的影响；对CAC率和积分与上述危险因素的关系在预测和预防临床冠心病中的作用进行评价。结果 高血压、高血脂、糖尿病和吸烟暴露组以及男性肥胖组的CAC率显著高于非暴露组(P<0.05)；男、女性有两种或以上危险因素同时存在组的CAC率和积分均显著高于无危险因素组和有一种危险因素组(P<0.001)。结论 高血压、高血脂、糖尿病、吸烟和肥胖对CAC率有重要影响；与单因素比较，多因素对CAC的影响呈倍数增加，且多因素的存在结合CAC积分在冠心病预测和预防中可起重要作用。

【Abstract】 Objective To explore the relationship between coronary artery calcification (CAC) detected by electron beam CT (EBCT) and risk factors of coronary heart disease (CHD) and its contribution to prediction and prevention of CHD. Methods Seven hundred and eighteen cases detected by Imatron C-150 EBCT scanner were grouped according to age, sex and the number of risk factors present in subjects. The relationship between the CAC incidence in men and women and the risk factors of CHD such as hypertension, hyperlipidemia, diabetes, family history and tobacco use was analysed. The influence of the number of risk factors in subjects on CAC incidence and score was also analysed. The contributions of them to prediction and prevention of CHD was assessed. Results The CAC incidence was significantly (P<0.05) higher in the patients with hypertension, hyperlipidemia, diabetes, family history, tobacco use both in men and women and obesity only in men than in the patients without them.
