【摘要】目的：探讨锁骨钩钢板治疗TossyⅢ型肩锁关节脱位的疗效。方法：采用手术锁骨钩钢板内固定治疗我院2003年10月～2008年5月收治的30例TossyⅢ型肩锁关节脱位的患者，观察治疗效果。结果：本组30例随访12～24个月，术后复查X线显示肩锁关节对位良好，8～12个月拔除内固定物后复查X线均未见复发脱位。27例患者肩关节无疼痛，无活动受限，术后3个月恢复正常工作生活; 3例患者肩关节活动时诉肩部疼痛不适，肩关节外展上举活动受限，拔除内固定物后，肩部疼痛症状消失，肩关节活动功能恢复正常。 结论：锁骨钩钢板治疗TossyⅢ型肩锁关节脱位具有固定牢靠、创伤小、手术时间短、能早期活动等优点，疗效可靠，应予临床推广。

Effect of clavicular hook plate on Tossy type Ⅲ acromioclavicular joint dislocation

[ABSTRACT] Objective: To discuss the effect of clavicular hook plate on Tossy type Ⅲacromioclavicular joint dislocation.Methods: A total of 30 cases with Tossy type Ⅲ acromioclavicular joint dislocation admitted from October 2003 to May 2008 were selected, and treated with internal fixation by clavicular hook plate. The therapeutical effect was observed. Results: A followup for 1224 months was carried out, and the Xray recheck showed that all acromioclavicular joint had good contraposition, and no reoccurrence occurred after fixer removal. Twentyseven cases had painless shoulder joint with unrestricted movement, and had normal life three months later after surgery. Three cases complained about pain in shoulder joint, and limited movement of shoulder abduction and uplift. The pain disappeared and the joint function recovered after fixer removal.Conclusions: With firm fixation, little trauma, short surgery duration, and fast recovery, clavicular hook plate is effective and reliable. It is worth extensive clinic application.

TossyⅢ型肩关节脱位，保守治疗效果差，必须行手术治疗，这在临床上已达成一致的观点。我院自2003年10月～2008年5月应用锁骨钩钢板内固定治疗TossyⅢ型肩锁关节脱位30例，取得了满意疗效，现报道如下。
1 资料与方法
1.1 一般资料
本组30例患者，男性21例，女性9例。年龄18～60岁，平均35岁。左侧脱位17例，右侧脱位13例，均为闭合性损伤，其中车祸伤12例，坠落撞击18例，损伤类型均为TossyⅢ型，伤后至手术时间6h～14d。
