B型脑钠肽测定在心力衰竭诊断和治疗中的作用

【摘要】  目的 研究B型脑钠肽(BNP)测定在充血性心力衰竭(CHF)诊治中的作用。方法 利用美国Biosite Diagnostics公司的B型脑钠肽试验测定300例心衰患者血浆中的BNP水平，观察各级心力衰竭患者BNP水平的变化;并且跟踪观察心衰患者在治疗过程中BNP水平的变化;同时测定20例非心衰患者BNP水平，进行统计分析。结果 心力衰竭患者血浆BNP水平有不同程度的增高，且增高的幅度与心功能不全的严重程度有关;在心衰患者治疗过程中，随着患者病情好转，BNP水平有不同程度的下降;患者病情恶化则BNP水平进一步升高，BNP水平升高提示患者预后不良。结论 血浆BNP水平可反映患者早期心功能受损的情况，对左心功能不全特别是充血性心力衰竭初步筛选、早期诊断、不同程度心衰患者评估、心衰治疗及疗效监测具有重要临床价值。

Role of B-type natnuretic peptide in diagnosis and cure of heart failure

　　[Abstract] Objective To evaluate the diagnostic value of B-type natnuretic peptide(BNP)for the patients of heart failure.Methods 300 patients of heart failure(HR)and 20 without heart failure patients(NHR)were selected and the value of BNP in plasma were determined by the way of biosite diagnostics，and the data were analyzed.Results To the patients of left ventricle dysfunction，the value of BNP in plasma augmented and had a correlative relation to the degree of dysfunction，in the progress of the therapy，as the deteriorated of the patient，the value of BNP would heighten，and if it looked up，the value of BNP would knock-down.Conclusion The BNP level in plasma reflected the function of heart to heart failure，to the congestive heart failure，BNP could lead a very important role in earlier diagnosis，evaluating of heart function and curative effect，and also could make a prognosis of the patient.

　　B型脑钠肽(B-type natnuretic peptide，BNP)，主要来源于心室，具有重要的病理生理学意义：促进排钠、排尿，具有较强的舒张血管作用，可对抗肾素-血管紧张素-醛固酮系统(RAAS)的缩血管作用，同ANP一样是人体抵御容量负荷过重及高血压的一个主要内分泌系统，反映心室功能改变有很高的敏感性和特异性。

　　充血性心力衰竭(congestive heart failure，CHF)按纽约心脏学会(the new york heart association，NYHA)分类将患者按严重程度分为Ⅰ、Ⅱ、Ⅲ、Ⅳ级。有报道CHF患者血浆BNP浓度较正常升高，与心衰严重程度呈正比，且比美国国家健康与营养检测积分和Framingham标准诊断CHF
