去除外骨膜对引导性骨再生模型成骨过程的影响 |

【摘要】 目的 研究去除外骨膜对引导性骨再生成骨过程的影响。方法 对24只新西兰兔双侧桡骨制作10 mm骨缺损，一侧保留外骨膜并用硅胶管连接，另一侧于缺损两端各去除外骨膜10 mm，其余手术方法相同，动物分别于术后3天和1、2、3、4、6、10、12周处死，标本行X线、非脱钙骨切片组织学检查。结果 （1）去除外骨膜后对于膜管内、膜管外成骨过程均无影响；（2）外骨膜生发层中的成骨细胞与哈弗系统中央管内及骨表面的成骨细胞相互联系；（3）外骨膜的两层结构分别来自于不同的组织：纤维层组织来自于外周的软组织，生发层中的成骨细胞来自于骨表面及哈弗系统。结论 去除外骨膜对引导性骨再生模型的成骨过程无影响。

【Abstract】 Objective To analyze the role of periosteum in guided bone regeneration by means of undecalcified sections and X-ray films. Method 10 mm long segmental defects were created in the diaphyses of both radii in 24 New Zealand rabbits. The defect on the control side was covered with a silicone membrane shaped as a tube. On the experimental side,10 mm periosteum of both sides of the defect was stripped and the defect was covered with a silcone tube. The animals were killed at the 3rd day and the 1st, 2nd, 3rd, 4th, 6th, 10th, 12th weeks. Samples were treated for radiologic and histologic examinations. Result (1) periosteal stripping exerts no effect on callus formation in or out of the tube. (2) osteoblasts in the cambium layer of the perioteumcome from bone surface, endosteum or the Haversian envelopes. (3) the tissue in the two layers of the periosteum has different origins, the fibrous layer comes from soft tissue and osteoblasts in the cambium from bone surface or Haversian envelopes. Conclusion Stripping periosteum has no influence on bone formation in guided tissue regeneration.

普遍认为外骨膜由生发层和纤维层两层结构组成，其生发层内含有成骨细胞，对长骨骨修复有促进作用。但对于外骨膜的作用还有许多争论，有学者认为外骨膜在骨折愈合过程中无显著作用［1］，而另有学者认为外骨膜是有成骨能力的独立、复杂的器官［2］。既往对外骨膜成骨作用的研究都是在外骨膜与各种成骨因素混杂的情况下进行的。而借助引导性骨再生模型可以使我们独立地研究外骨膜的成骨作用。我们应用引导性骨再生的动物模型，以非脱[1] [2] 下一页
