依托咪酯乳剂在老年人诱导麻醉中的脑电双频和循环的变化 |

摘要 目的：采用脑电双频谱指数（BIS）评价依托咪酯乳剂在老年人麻醉诱导时的深度及循环的变化。方法：20例ASAⅠ～Ⅱ级腹腔镜胆囊切除术患者，年龄大于60岁（n＝10）为老年组，小于60岁（n＝10）为对照组。麻醉诱导用药为芬太尼2ug／kg、依托咪酯0.3mg／kg、阿曲库铵0.5mg／kg。结果：老年组最低BIS为43.1±6.4，对照组为38.4±4.7，无显著差异。达最低BIS所需时间老年组为43.5±7.4秒，对照组为35.6±8.1秒（P＜0.05）；老年组用药前后的BP、HR稳定，对照组心率明显下降（P＜0.05）。结论：依托咪酯乳剂用于老年人麻醉诱导是安全的。

The Changes of Bispectral Index and Circulation during Induction
of Anesthesia with Emulsion of Etomidate in Aged Patients

Abstract Object：To evaluate the changes of BIS and circulation following induction of anesthesia with emulsion of etomidate in aged patients．Method：Twenty patients（ASAⅠ-Ⅱ） undergoing elective cholecystectomy were divided into groups A（age＞60，n＝10） and C（age＜60，n＝10）．Fentanyl 2μg／kg etomidate 0.3mg／kg and atracurium 0.5mg／kg were administered i．v．for anesthesia induction．Result：The lowest BIS was 43.1±6.4 in group A and 38.4±4.7 in group C（P＞0.05）．The onset time of lowest BIS was 43.5±7.4s in group A and 35.6±8.1s in group C（P＜0.05）．The BP and HR of group A was stable，while HR was significantly decreased in group C（P＜0.05）．Conclusion：Induction of anesthesia with emulsion of etomidate is safe in aged patients．

脑电双频谱指数（BIS）用于麻醉深度的监测已被广泛应用。依托咪酯乳剂作为一种新型制剂，在临床上应用报道较少。本研究观察依托咪酯乳剂在老年人麻醉诱导时的BIS及循环的变化。
资料与方法
一般资料 20例ASAⅠ-Ⅱ级腹腔镜下胆囊切除术患者，大于60岁10例为老年组，小于60岁10例为对照组。老年组中，男2例，女8例，平均年龄65.4±3.06岁（60～70岁），体重56.5±9.95kg（44～77kg）；对照组中，男3例，女7例，平均年龄42.1±8.7岁（27～57岁），体重56.9±10.1kg（44～69kg）。所有病例均无中枢神经系统病变及术前应用影响中枢神经系统的药物。
麻醉方法 术前30分钟肌注哌替啶50mg、异丙嗪25mg。病人入室安静5分钟后记录BIS、HR、SBP、DBP的基础值。麻醉诱导用药为芬太尼2μg／kg、依托咪酯0.3mg／kg、阿曲库铵0.5mg／kg，依托咪酯推注时间为30秒，[1] [2] 下一页
