自体骨髓干细胞移植治疗系统性红斑狼疮的临床研究 |

【摘 要】 目的 探讨自体骨髓干细胞移植(ABMSCT)对系统性红斑狼疮(SLE)的疗效。方法 采集自体骨髓移植CD34＋造血干细胞，预处理用环磷酰胺(CTX)60 mg/kg连续2天静滴，马法兰140 mg/m2分次口服，用格拉诺赛特(G-CSF)刺激粒细胞的恢复，用间接免疫荧光法检测抗核抗体、放免法测抗DNA抗体、流式细胞仪测淋巴细胞亚群，观察ABMSCT前后临床表现和免疫学指标的变化。结果 ABMSCT后患者的临床症状明显缓解，异常免疫学指标恢复正常，抗体全部转阴。结论 ABMSCT对SLE有较好的疗效，远期疗效还需长期随访。

【Abstract】 Objective To investigate therapeutic effect of autologous bone marrow stem cell transplantation (ABMSCT) on sytemic lupus erythematosus (SLE).Methods Transplantation of haematopoietic stem cell CD34＋ acquired by autologous marrow aspiration was adopted.The conditioning before transplantation consisted of 60 mg/kg intravenous cyclophosphamide (CTX) for two days and 140 mg/m2 oral maphalan.Granulocyte recovery was stimulated by G-CSF.Indirect immunofluorescent and radioimmunologic tests used to examine ANA and anti-DNA.Lymphocyte subset was analyesd by flow cytometry.Both clinical manifestations and immunologic markers were compared before and after transplantation. Results An excellent clinical and immunologic remission was observed post-transplant and immunologic abnormality returned to normal.All antibodies were nagtivisation.Conclusion The therapy of autologous ABMSCT for SLE is remarkably effective.It is required to follow-up for evaluation long effect.

系统性红斑狼疮(SLE)是一以多脏器受累和血中存在多种抗体为特征的自身免疫病。造血干细胞移植(HSCT)治疗自身免疫病是90年代以来国外研究的热门课题。动物实验结果证实HSCT后狼疮鼠的抗体滴度下降、肾功能改善；同基因骨髓移植(BMT)对BALB/c小鼠的实验性抗磷脂抗体综合征(APS)同样有效〔1〕。其他动物试验也发现不论是同基因还是异基因HSCT，对动物的佐剂性关节炎和实验性过敏性脑脊髓膜炎(EAE)都有一定的疗效〔2〕。意大利学者Marmont等首先报告自体骨髓干细胞移植(ABMSCT)治疗一例病程较长且病情严重的SLE患者，疗效显著〔3〕。我院于1998年10月对一例药物治疗不好的SLE患者成功进行了ABMSCT，获得了满意的疗效。
