【摘要】 目的：探讨体外培养经皮移植自体骨髓间充质干细胞(MSCs)治疗四肢骨折骨不连的效果。方法：12例四肢骨折骨不连患者采用体外培养经皮移植自体MSCs治疗。结果：9例获得了骨性愈合，骨折愈合平均时间为5个月(4～7月)，X线显示骨折线消失，骨痂形成。骨折愈合率为75%(9/12)。结论：体外培养经皮移植自体MSCs治疗四肢骨折骨不连创伤小、安全、并发症少，临床效果满意。

Abstract: Objective To explore the efficiacy of percutaneous transplantation with autologous marrow mesenchymal stem cells cultured in vitro on the treatment of bone nonunion.Methods Twelve patients with bone nonunion were treated by percutaneous transplantation of autologous mesenchymal stem cells cultured in vitro.Results The rate of fracture union was 75%(9/12),the average fracture union time was 5 months(4～7 months),Xray demonstrated that the fracture line nearly disappeared and porosis increased.Conclusion Percutaneous transplantation of autologous marrow mesenchymal stem cells cultured in vitro for treatment of bone nonunion may be a less traumatic,safe,less complication and excellent clinical outcomes method.

目前四肢骨折骨不连大多采用外科手术切开植骨内固定治疗，但外科手术治疗方法存在创伤性和危险性较大、费用高等问题。随着骨髓间充质干细胞(mesenchymal stem cells，MSCs)的研究深入，采用自体MSCs移植治疗骨不连取得了一定的效果。本课题的前期实验研究部分证实腺病毒介导的BMP2基因转移可以促进人间充质干细胞成骨化[1]。动物实验也发现骨髓中的干细胞浓度与骨髓成骨能力存在正相关[2]，说明MSCs在骨折的愈合过程中起重要作用，但临床应用的效果如何则报道不多。现对我科12例四肢骨折骨不连患者采用体外培养、扩增经皮移植自体MSCs治疗的效果进行评估。
1 资料与方法
1.1 临床资料
收集2005年6月-2008年3月四肢骨折骨不连病患12例，男8例，女4例。年龄20～48岁，平均年龄32岁。病程8～30月，平均12月。所有病例均符合骨不连诊断标准(骨折后6个月不愈合，骨折后3～6月连续观察骨折无进行性修复，无愈合迹象)。骨不连部位：肱骨2例，前臂尺桡骨2例，股骨1例，胫骨7例。初始治疗方法：石膏外固定2例，切开复位钢板内固定10例。适应症：骨折端间隙小于5 mm;无骨折端成角、短缩及旋转移位;非感染性骨不连者。
