诱导性一氧化氮合酶抑制药对感染性休克治疗作用的观察

摘要 目的:评价诱导性一氧化氮合酶(iNOS)抑制药氨基胍(AG)的抗感染性休克作用。方法:杂种犬(8~12kg)2小时内静滴内毒素(LPS,1mg/kg)后随机分为对照(n=6)和治疗组。治疗组在MAP下降后2小时分别静注L-N-位硝基精氨甲酯(L-NAME,n=6,30mg/kg)和AG(n=6,20mg/kg)。结果:静注LPS后1小时在NO升高同时MAP明显下降,SVR和尿量明显减少。给予L-NAME后,MAP、MPAP、CVP和SVR明显升高,CO和HR明显减少(P<0.05),尿量无明显增加。静注AG后MAP、SVR和尿量明显增加(P<0.05),而CVP、PAP、CO和HR无明显改变。结论:感染性休克中NO明显增加,AG对循环功能的改善好于L-NAME。

Abstract Objective:To evaluate the effects of aminoguanidine(AG),a selective inhibitor of the inducible isoform of nitric oxide synthase(iNOS) in septic shock.Method:After a continuous infusion of LPS(1mg/kg) for 2 h,mongrel dogs were divided into control(n=6),L-NAME (n=6) and AG (n=6) groups.Two hours after the MAP decreased to 60% of the baseline,L-NAME(30mg/kg)or AG(20mg/kg) was given intravenously in bolus,respectively.Result:One hour after LPS injection,the blood concentration of NO began to increase significantly,and simultaneously MAP decreased by 40% ,SVR and urine output(UO) also decreased.After injection of NAME,MAP,SVR CVP and PAP increased significantly,NO,CO and HR decreased significantly (P<0.05),UO was kept at the low level.After the injection of AG,MAP,SVR and UO increased significantly,CVP,CO,HR and PAP had little changes.Though the concentration of NO decreased significantly,its extent was much less in AG group than that in L-NAME group.Conclusion:The blood concentration of NO may increase in septic shock.AG only inhibiting the iNOS has better effects than L-NAME in improving the hemodynamics.
