替比夫定联合复方五仁醇胶囊治疗慢性乙型肝炎的疗效观察

　　【摘要】 目的 观察替比夫定联合复方五仁醇胶囊治疗慢性乙型肝炎的疗效。 方法 将56例慢性乙型肝炎患者随机分为治疗组30例，对照组26例，分别应用替比夫定联合复方五仁醇胶囊治疗和替比夫定治疗50周以观察其疗效。 结果 治疗组和对照组谷丙转氨酶复常率分别为86.67%和57.69%，HBeAg阳性转阴率分别为33.33%和15.39%，HBV-DNA转阴率为80.00%和61.54%，差异有显著性(P<0.05)。 结论 替比夫定具有抗乙型肝炎病毒特异性，与复方五仁醇胶囊联合抑制病毒能力较单用替比夫定强，乙型肝炎病毒转阴率略有增高，谷丙转氨酶复常率明显增高，HBeAg血清转换率小幅增高，安全性较好。

　　Efficacy of telbivudine and compound wurenchun capsule in treatment of chronic hepatitid B

　　[Abstract] Objective To investigate the efficacy of telbivudine and compound wurenchun capsules in the treatment of chronic hepatitid B. Methods A randomized, open-label trial was conducted. 56 cases of chronic hepatitis B were assigned to two groups. 30 cases received the combination treatment of telbivudine and compound wurenchun capsules, 26 cases were treated with telbivudine only.All cases received treatment for 50 weeks.Results The normalization rates of biochemical markers alanine aminotransferase were 86.67% and 57.69% respectively，and rate of seroconversion from hepatitis B e antigen to hepatitis B e antibody were 80.00% and 61.54(P<0.05). Conclusion Telbivudine has potent virus suppression. Telbivudine combined with compound wurenchun capsules can raise the efficacy of normalization of ALT and seroconversion of hepatitis B to e antibody, and the combination therapy is a safety protocol.

　　我国每年死于乙型肝炎相关疾病的约30万例，所以对乙型肝炎的治疗尤其重要。本实验采用替比夫定联合复方五仁醇胶囊或单用替比夫定治疗慢性乙型肝炎的疗效的观察，研究两种治疗方法之间的差异及疗效。

　　1 资料与方法

　　1.1 病例选择 选择2008年5月—2009年4月本院肝病科收治的慢性乙肝患者56例，男41例，女15例，年龄31～65岁。患者符合以下标准[1]：(1)谷丙转氨酶反复异常，较正常值上限升高2～5倍。(2)乙型肝炎病毒检测持续阳性>2年，近2年来未服用过抗病毒药物。(3)HBeAg持续阳性>3年。排除年龄>70岁，孕妇及哺乳期妇女，合并心脏病、肾病、活动性肺结核、自身免疫性疾病、遗传病的慢性乙型肝炎的患者。两组患者的年龄、身高、体重、性别方面的差异无统计学意义(P>0.05)，具有可比性。
