高同型半胱氨酸血症与冠脉病变的关系 |

【摘 要】 目的 了解血清同型半胱氨酸(Hcy)水平与冠脉病变的关系。方法 用断面调查的方法收集经冠状动脉造影确诊的冠心病患者102例、 非冠心病患者52例，用高效液相色谱测定血清 Hcy 水平。 结果 冠脉病变与男性、吸烟、糖尿病及血清Hcy升高明显相关；冠心病组 患者Hcy水平［(19.68±10.05)μmol/L］明显高于非冠心病组［(11.33±4.17)μmol/L ］，P＜0.01。冠心病合并糖尿病组冠脉病变程度较对照组明显严重。多元Logistic回 归分析证实Hcy和糖尿病的比势比(OR)均大于1，并有显著差异。用前进法观察偏回归系数证 实这两个因素是冠脉病变的独立危险因素。 结论 血清Hcy升高和罹患糖尿病是导致冠脉病变的重要而各自独立的危险 因素。

Significance of serum homocysteine in patients with coronary heart diseases.

【Abstract】 Objective To investigate the rel ationship between serum homocysteine (Hcy)and atherosclerosis.Methods Serum Hcy levels of 102 cases with coronary he art diseases(CHD) and 52 non-CHD subjects were measured using high performance l iquid chromatography(HPLC)with fluorescence detection.Results In a logistic regression analysis,the extent of coronary atherosclerosis was correspondingly related with sex,smoking,diabetes and Hcy level.The mean serum Hcy level was significantly higher in coronary pati ents than in controls(P<0.01).The patients with both diabetes and high lev els of Hcy had more severe coronary atherosclerosis than in controls.By multivar iate analysis, the elevated Hcy level and diabetes were two independent factors in coronary atherosclerosis.Conclusion Hcy level and diabetes were two independent risk fact ors in the development of atherosclerosis.

众所周知，冠心病的发生与很多因素有关，其中性别、高血压、高血脂及糖尿病等均 被视为主要危险因素。近年来发现同型半胱氨酸(Hcy)与冠心病的发生密切相关。本研究通 过测定血清Hcy，了解它在冠心病发病中的意义。
1 对象与方法
该研究收入1998年4～10月间我院心内科住院行冠状动脉(冠脉)造影术的154例病人。按 冠脉造影结果分为冠心病组和非冠心病组，冠心病组判断标准为冠脉造影结果至少1支以上 病变狭窄程度≥50%，反之判为非冠心病组；其中冠心病组共102例，平均年龄为37～74岁(5 8.3±9.4岁)，非冠心病组52例，平均[1] [2] 下一页
